Note: If any sample is positive for asbestos, no ACP-5 will be issued. Instead, a full lab report will be provided so you can plan next steps or arrange abatement.

What Is an ACP-5?
An ACP-5 is a DEP asbestos compliance form confirming no asbestos was found. It’s required for most DOB permits.
We only issue an ACP-5 when all samples are confirmed negative. If any material tests positive, you’ll receive the full report — but ACP-5 cannot be filed until abatement is done and the area is retested.
